(Helping Eradicate All Roads To Homelessness)

· Are you or is someone you know at risk of losing, about to lose, or have already lost your housing?

The Tri-County (Jefferson, Lewis and St. Lawrence counties) HEARTH Program can help with rent, utility bills, moving costs, security deposits, and some legal fees. Each eligible person must be at or below 50% of County Median Income (click here for the income limits page) and must be affected by one or more of the following factors:

· Eviction pending within two weeks

· Sudden/significant loss of income

· Sudden/significant increase in utility costs

· Credit problems

· Housing costs are more than 50% of income

· Families with children under two years old

· Young head of household (under 25 with children or pregnant)

· Living in housing that has been condemned

· Recent traumatic life event

· Multiple families in a single-family home

· High overcrowding

· Overcrowded household

· Mental health and/or substance abuse issues

· Physical disabilities and/or chronic health issues (including HIV/AIDS)

· Have been homeless within the last 12 months

· Significant amount of medical debt

· Extremely low income (30% of median)

· Pending foreclosure of rental housing

· Currently experiencing a housing crisis

· Past institutional care

· Eviction from public or assisted housing

· Involvement in child welfare, including foster care

· Discharge within two weeks from an institution where the person has been a resident, including prisons, mental health institutions, and hospitals
